HOW TO APPLY | DSWD REGION VII
Interested and qualified applicants are instructed to submit their applications with complete set of documents:
1. Application Letter indicating the position applied for and addressed to:
Regional Director Shalaine Marie S. Lucero,
CESO IV of DSWD Field Office VII
(specifying the position applied for and its date of publication)
2. Duly Accomplished Personal Data Sheet (PDS) with recent passport-sized picture and signature and Work Experience Sheet
3. Updated and properly accomplished Work Experience Sheet (CS Form 212 Attachment); if any
4. Photocopy of Transcript of Records ( Authenticated)
5. Photocopy of authenticated civil service eligibility/ Professional License; if any
6. Photocopy of Transcript of Records/ Diploma
8. Photocopy of Relevant trainings and Seminars attended
9. Photocopy of Performance Rating in the last rating period (for government employees) or its equivalent for external applicants
