2. CSC properly accomplished and notarized Personal Data Sheet (PDS) with recent passport-sized picture (CSC Form 212 Revised 2017) and accomplished Work Experience Sheet (CS Form 212 Attachment) which can be downloaded at www.csc.gov.ph;

3. Resume

4. Photocopy of Transcript of Records (Authenticated);

5. Photocopy of Relevant Certificate of Trainings and Seminars attended;

6. Certificate of Board Rating/CS Eligibility (Authenticated if any);

7. Photocopy of Certificate of previous employment (if any);

8. Copy of duly signed IPCR/Summary of Ratings (for insiders only);
